COVID- 19 Update

We are currently booking our initial appointments via Telephone or virtually which ever you prefer.  If your Nurse Practitioner requires you to be seen in person a second appointment will be scheduled to be seen for an in-person visit.  If you are scheduled for an in-person visit, please come for your scheduled appointment time as our waiting room is currently closed. We ask that you read the screening questions carefully at the door and request you to change your mask to one of our medical grade masks and your temperature will be taken.

If you have paperwork to drop off you can utilize our secure drop box on the front of our building. Ensure your information has been completed including any spots you are required to sign or we will request you return to complete these.

If you are running out of medication, please ask the pharmacy to fax the renewal request to the Smiths Falls Nurse Practitioner-Led Clinic. Our policy is that you allow 7 days for renewal requests to be completed.

Nurse Practitioner-Led Clinics are a collaborative team based health care model in which Nurse Practitioners are the lead providers of primary health care. These clinics were created to provide accessible, quality family health care services to people that are unattached in their community.
