About Us

 Better Health Together.

We are a team-based primary care clinic serving clients of all ages across Smiths Falls, Lanark, Leeds, and Grenville. Funded by Ontario Health, focused on your health. 

Our Story

Where we started and why we're here.

In 2010, our community needed something different. Too many people were bouncing between walk-in clinics and emergency rooms, or trying to manage complex health needs without the coordinated support they deserved. That year, Smiths Falls Nurse Practitioner-Led Clinic was approved as the first and only Nurse Practitioner-Led Clinic in Lanark Leeds and Grenville.    

We opened our doors in May 2012 with the commitment to provide quality primary care for people without a healthcare provider.   
More than a decade later, we're still here. Still growing. Still committed to our community. We are funded by Ontario Health through the Ministry of Health and Long-Term Care - which means every decision we make is about the people we serve and high-quality care.

Our Approach

A team working for your health.

Your Nurse Practitioner doesn't work alone. At the Smiths Falls NPLC, your care is supported by Registered Nurses, a Registered Dietitian, a Registered Social Worker, a Pharmacist, and a dedicated administrative team - collaborating for you.

Registered clients can access any member of our team by calling reception. No referral needed.

Nurse Practitioners

Our Nurse Practitioners lead your primary care. They assess, diagnose, treat, prescribe, and coordinate your care - working closely with the other members of the team.

Registered Nurses

Our Registered Nurses work at the full scope of practice. They conduct health assessments, meet-and-greet visits,  immunizations for all ages, perform well-baby and prenatal visits, and provide health education, as well as cervical screening and  chronic disease visits for things such as the blood pressure monitoring program.

Registered Dietitian & Collaborative Care Services

The Registered Dietitian provides practical nutrition advice for people who want support with weight management, blood pressure tracking, cholesterol, blood glucose levels, or general questions about healthy eating and lifestyle and wellness.

Pharmacist
The Pharmacist helps clients get the most from their medications, including reviewing the medications you take, identifying drug interactions, making recommendations on medication adjustments, and providing education related to your chronic conditions - with monitoring and follow-up. 

Administrative Staff

Our administrative team is often the first voice you hear and the first face you see. They manage appointment booking, coordinate between team members, manage incoming referrals and faxes, and serve as the connection between you and the care team. They keep the clinic running smoothly. There is a great deal that happens behind the scenes - and our administrative staff make all of it possible.

Governance

Board of Directors

The Smiths Falls NPLC is governed by a Board of Directors with 51% Nurse Practitioners and 49% community members. The Board provides strategic oversight and is accountable to the community we serve. Day-to-day business operations are managed by our Executive Director, who oversees the administrative functions, privacy, and works with the NP co-Leads who oversee clinical day to day operations.
