This page is organized into two sections. The first covers programs and resources offered directly through the Smiths Falls NPLC. The second is a list we have created of community services and supports across Lanark, Leeds, and Grenville; organized by category to make them easy to find.

Clinic Programs

Programs offered through Smiths Falls NPLC.

ConnectWell - Lung Health Program
ConnectWell - Lung Health Program

Our clinic connects clients with the ConnectWell Lung Health Program, which supports people living with asthma and COPD. This program offers education, monitoring, and self-management support.

Immunization Clinics
Immunization Clinics

We provide all routine immunizations as recommended by Southeast Public Health, including catch-up immunizations for adults and children. Flu immunization clinics run throughout the fall and winter months.

Single-Session Counselling
Single-Session Counselling

A single meeting with our Registered Social Worker at the time you need it most. Come prepared with what you'd like to get out of the session - together you'll develop a clear plan and practical steps forward. Available in person, by telephone, or virtually.

This is not a crisis service. For immediate support, contact 9-1-1 or your nearest emergency department.

Nutrition Counselling
Nutrition Counselling

Individual nutrition counselling with our Registered Dietitian for weight management, blood pressure, cholesterol, blood glucose levels, and general healthy eating questions.

Health Education Sessions
Health Education Sessions

One-on-one and group education on prevention, wellness, and chronic disease management. Topics and schedules vary — follow us on social media or ask reception to see what's currently available.

Upcoming Programs

Education sessions and workshops.

Current offerings through Living Well - South East Ontario:

We offer and connect clients with group education sessions and workshops on a range of health topics. Registration is often free and no referral is needed.

Registration is limited. No cost and no referral needed.
Open to anyone in Southeast Ontario. Funded by the Ministry of Health and Long-Term Care.
